Gynaecomastia refers to the condition where males develop breasts. This can vary from very minor lumpiness just beneath the nipple to fully developed feminine breasts.

This is a benign condition that commonly occurs during pubescence in approximately 60 to 70% of healthy males between the ages of 12 and 15. In most of these cases the Gynaecomastia disappears.

Adult Gynaecomastia can affect as many as 30% of men and whilst most causes are unknown, there are multiple possible causes that need to be explored. These include a persistence of pubescent gynaecomastia, obesity, steroid abuse, endocrine disorders and certain drugs. The surgery required to correct this problem depends on the severity of the condition.

What is Gynaecomastia?

Gynaecomastia is a condition in which male breast tissue becomes enlarged. This can range from minor swelling just beneath the nipple, to more feminine looking breasts. It’s essential to know that Gynaecomastia is generally physically harmless and can often occur during puberty in healthy males. In most cases, this condition resolves on its own. However, adult Gynaecomastia can affect a reasonable proportion of men and has various causes, including hormonal imbalances, obesity, steroid use, endocrine disorders and a side effect of certain medications.

Understanding the causes of Gynaecomastia

The development of gynaecomastia can either be due the abnormal development of breast tissue alone, or also due to the development of excess breast associated fatty tissue. It could be either mostly breast tissue, mostly fatty tissue, or often a combination of the two. Different factors can cause gynaecomastia. To distinguish between the two, you will require a thorough examination with Dr Dona to determine the severity and cause. The different Gynaecomastia Surgical procedures

Gynaecomastia Surgery is often conducted as a day hospital procedure, which means you can go home on the same day of the surgery. However, larger corrective cases may warrant staying in hospital overnight.

The type of surgery depends on the severity of Gynaecomastia.

Liposuction/Liposculpture

When excess fat is mainly responsible for gynaecomastia, Liposculpture might be a suitable option. In this procedure, small incisions (one or two on each breast approximately 5mm) is made near the treatment area. Through these incisions, a thin tube called a cannula is inserted. The cannula helps break down and remove excess fat deposits from the chest. This surgery is treating the gynaecomastia by targeting and eliminating excess fatty tissue deposits. The end result is a chest that appears more masculine with less prominence of the breasts.

Liposuction alone for a primarily fatty breast related gynaecomastia would be the first choice when the person has no significant excess skin.

Surgical Excision

When Gynaecomastia involves a significant amount of glandular tissue or is more severe, surgical excision becomes the preferred choice. A number of surgical patterns are possible depending on the severity.

The commonest approach involves a single scar along the lower areolar border. This typically heals extremely well. These incisions provide access to the underlying tissue. Dr Dona then surgically removes the excess glandular tissue and fat. The primary aim is to create a chest with a flatter and more masculine appearance.

In instances where there is a noticeable excess skin, additional incisions beyond the areolar border might be required to remove the extra skin to achieve an optimal outcome.

For those men that have fully developed and even pendulous breasts, a formal breast reduction operation similar to that which a female would require may be necessary. Therefore, the scar patterns required will be similar.

Unlike a female breast reduction where retaining breast tissue is typically required, this is not the case in gynaecomastia surgery. Without getting too technical, this would often mean that in a large gynaecomastia case the nipple/areola may need to be surgically removed and then grafted into the appropriate anatomical position.

Both Liposculpture and surgical excision are surgical methods designed to address the concerns related to gynaecomastia, but the choice between them depends on the specific characteristics of your condition, such as the proportion of fat to glandular tissue and the degree of skin excess. Immediate postoperative period

Immediately following the procedure, you should be taken to a recovery area where your condition should be closely monitored until you awaken and stabilise. Most patients can return home on the same day as the procedure but be prepared to experience some pain, discomfort, swelling and bruising to the chest area.
